We accept any one of the following:
A photo or scanned copy of a valid prescription
Must clearly show:
• Your full name
• Medication name and dosage
• Prescriber's name and details
• Date of issue
A prescription label from prior packaging
Should include your name, the dispensing pharmacy, and medication information.
A recent medication summary or patient record
Issued by a GP, clinic, or digital health platform.
A letter or email from your prescriber
Must confirm your use of the medication and be issued within the past 12 months.

You can submit your prescription evidence in the following formats:
Images: JPG, JPEG, PNG (clear, well-lit photos)
Documents: PDF (scanned letters, summaries, or prescriptions)
Screenshots: From your patient portal or prescription history (as long as they are clear and legible)
Please make sure:
Files are no larger than 10MB
Text is readable and not cropped or blurry
All required details are visible
